Assessing the nitric oxide efficacy in bilateral lung transplantation

چکیده
Background . One of the most frequent and severe complications in early postoperative period lung transplantation is primary graft dysfunction resulting from ischemia-reperfusion injury. There evidence effectiveness using inhaled nitric oxide order to prevent such Objective To assess intra- bilateral transplantation. Material methods We examined 43 patients who underwent at N.V. Sklifosovsky Research Institute for Emergency Medicine 2012-2021. The were divided into two groups. study group consisted 23 patients, whose complex treatment included use oxide. Patients comparison (n=20) received a standard therapy. end points were: mechanical ventilation duration, frequency extracorporeal membrane oxygenation its mortality, dynamics index, blood lactate level, pH, base deficiency. Results therapy during improved ventilation-perfusion ratio, as evidenced by an increase index 1.1 times (p=0.128) 1.3 (p=0.026) 48 72 hours after surgery, respectively. Meanwhile, surgery was found decrease 1.2 (p=0.033), that decreased 1.4 (p=0.474); duration (p=0.042); 1.6 (p=0.028); mortality reduced 8%. Conclusion had positive effect on period, indicated improvement gas parameters, reduction veno-arterial oxygenation, ventilation.

The therapeutic potential of nitric oxide in lung transplantation.
Endogenously produced oxides of nitrogen appear to play important roles in tissue and organ homeostasis. Endogenous production of nitric oxide, which can be altered in response to various stimuli, can modulate vascular tone, oxyradical cascades, cell adhesion, and other aspects of inflammation. Because exogenously administered (inhaled) nitric oxide can mediate pulmonary vasodilatation and impr...

BACKGROUND Therapies for neonatal chronic lung disease (CLD) of prematurity have had limited success. AIMS To determine whether inhaled nitric oxide (INO) administered to very low birthweight infants with developing CLD might improve oxygenation without adverse effects. METHODS Subjects were 10-30 days of age, birth weight < 1250 g, with developing or established CLD, and requiring mechanic...
متن کاملNitric oxide in cardiac transplantation.
The endothelium of coronary arteries has been identified as an important organ locally regulating coronary perfusion and cardiac function by paracrine secretion of nitric oxide (NO) and other vasoactive mediators. Therefore, the established organ procurement in cardiac transplantation using hypothermic storage solutions designed to preserve myocytes but not endothelial cells has to be criticall...
متن کاملExhaled nitric oxide after lung transplantation: impact of the native lung.
It has already been demonstrated that exhaled nitric oxide (eNO) is increased in lung transplant patients with chronic rejection, although it is not known whether the diseased native lung after single lung transplantation (SLTx) contributes to the increased eNO values. This study aimed to compare the eNO values in stable lung transplant patients (SLTx versus sequential (S)SLTx and heart (H)LTx)...
متن کاملNitric oxide production in the hypoxic lung.
Nitric oxide (NO) is a potent vasodilator and inhibitor of vascular remodeling. Reduced NO production has been implicated in the pathophysiology of pulmonary hypertension, with endothelial NO synthase (NOS) knockout mice showing an increased risk for pulmonary hypertension. Because molecular oxygen (O2) is an essential substrate for NO synthesis by the NOSs and biochemical studies using purifie...
